Gernsback actually had an even earlier representation of Telemedicine in April, 1924 in his "Radio News" magazine. He called it "The Radio Doctor".

Love this. I frequently speak and write on the tech and medicine. I would love to use these images in lectures, etc. Is there anyone who can give me info on copyright?

The 'remote pen' bit is also a hit:

The LongPen, invented by Margaret Atwood, is pretty much exactly what is shown here. Including seeing and talking to people at the other end.

She invented it for situations where she couldn't attend a book signing in person.

Actually, there is a practical household form of such a device. It is a Danish project (http://www.viewcare.com/ Danish only). The NY Times had an article on it in 2010 already (http://www.nytimes.com/2010/01/12/health/12denmark.html), and since then the project has gained a high reputation and a growing number of cooperating practitioners and hospitals. The radio transmission is replaced by a reliable Internet connection (which is the key aspect for usability at home in the end) provided by a quite unique multichannel bonding solution by a German company (www.viprinet.com)
